CONSUMER RIGHTS NOTICE

Consumer Rights & Required Disclosures Texas Licensed Professional Counselor (LPC)

This practice complies with Texas House Bill 4224 (89th Regular Session), effective September 1, 2025, and Section 181.105 of the Texas Health and Safety Code.

Texas law requires licensees to prominently post instructions informing consumers of their rights. Those instructions are provided below.


Requesting Your Health Care Records

You have the right to request access to or copies of your health care records maintained by this practice.

Instructions: Submit a written request directly to this practice. Requests may be made in person, by email, or by mail. Valid identification may be required prior to the release of records.

For questions regarding licensure, regulation, or professional standards, you may contact the Texas Behavioral Health Executive Council (BHEC).

Filing a Consumer Complaint
